Digital Requisition Forms are accepted via fax + email. This form can also be printed and brought in manually. They must however be completely filled out, and signed by the ordering physician.
ChemScreen Laboratory Services Inc. requisition forms come with a carbonless duplicate. However digital forms are autofill. You can fill it out digitally, download and then share as instructed below. It is important that samples are accompanied by the correct requisition form, completed in its entirety. To email a patient’s requisition, please follow the steps below:
