Completing and Sending Forms

ChemScreen Laboratory Services Inc. requisition forms come with a carbonless duplicate. However Digital forms are autofill. You can fill it out digitaly, download and then share as instructrd below. It is important that samples are accompanied by the correct requisition form, completed in its entirety. To email a patient’s requisition, please follow the steps below:

Email

1. Send one email per patient
2. Email a PDF copy of the requisition to the email address below:

Fax

1. Send one fax per patient
2. Use the patient’s legal first name and last name as the subject line
3. To fax a patient’s requisition, please use the following fax number:

The patient is responsible for the security of the electronic copy of the requisition when it is on their mobile device or when it is emailed from the patient’s public email domain to ChemScreen Laboratory Services.
