How is a drug test requested?

• Referral from doctor
• Referral from drug counsellor or drug  support agencies

What type of sample is used?

• Urine samples may be transported from the referral agency/ doctor or collected at our facilities.
• For alcohol test a urine sample or a blood sample is required.

Identification

• For collection at our facilities the following is required:
  
  • Photo identification for all individuals over 16 years of age.
  • Identification card of children under 16 OR
  • Positive identification by a donor representative (Doctor, Agency   representative, Counsellors)

All minors under 18 must be accompanied by an identified parent/ guardian or donor representative.
